- Not Insurance: This is an In-House Membership and not an insurance and is not a qualified health plan. It may not be combined with insurance, other offers, or discounts.
- Enrollment & Fees: Membership fees are due at the time of enrollment and expire 1 year from the date of enrollment. Enrollment is available on a yearly basis only. Annual membership price is subject to change.
- Payments: All services rendered must be paid in full on the date of service to receive these discounts; otherwise, you will be billed full price.
- Exclusions & Limits:
- Services are non-transferable and non-refundable.
- Any services not used within the enrollment period cannot be carried over.
- Discounts cannot be applied to any specialist to whom we may refer.
- All fees shall be based on our practice’s usual, customary, and reasonable fees, which are subject to change without notice. Some exclusions may apply.
